A certificate noting the name of the child and parent, the date and the registration district. Instructions at the bottom detail the parent's obligation to comply.
This copy is filled out to Mr A G Hill, the father of Clement Alfred Hill. It is dated at Dunedin on 16 October 1903.
Donor reports that the vaccination would have been for smallpox, in those days (See letter at AT 10/21, 20 November 2000).
