Certificate was not transferable. It shows the coat of arms of the Dominion of New Zealand at top centre, and is printed with green ink on cream paper, with a narrow ornate border around the text. It is countersigned at the left, by H[?] S Greig on 14 August 1939.
The verso has a grid of 12 spaces for the paying officer's stamp, to be filled in when a payment is made to the beneficiary. Five of the spaces are filled in, for dates in August - November 1939.
